Centracom II Manual

Looking for the cap code conversion chart that is in the back of the Centracom II service manual. We misplaced our manual and need to add a tone. I did a search and found some indusrty standard charts which isn't what I'm looking for. This was the actual Motorlola chart in the back of the service manual. It would convert your tones and sequence to something like this: AE*#F#
